Young carers support for children and young people aged 5-18 and for sibling carers (i.e. children who help care for and/or are impacted by a sibling with additional needs) aged 5-18.
This to be achieved through the provision of an individually tailored package of support based on a statutory assessment of need and outcomes (completed by the Local Authority Young Carers Assessment Worker) and agreed by the young carer and their family.
Support will vary dependent on needs but will include one-to-one support, accessible group activities, an overnight residential stay per annum and issue-based workshops. Service delivery plans to be reviewed regularly.
This service will strive to ensure that individual carers are:
1. Helped to optimise their physical and emotional wellbeing.
2. Supported to relieve the stress of the caring role.
3. Able to maintain social contacts and personal relationships.
4. Assisted to enjoy separate social and community activities.
5. Enabled to participate in leisure activities and education
The contract will be for a period of 3 years, with an option to extend for up to 1 year
